Are you ready for your minor body piercing appointment with Pens & Needles?
For any piercing of a minor, a PARENT or LEGAL GUARDIAN:
- must complete the consent form below and attach a State-issued photo identification from the legal guardian
and a form of identification from the minor (driver’s license or state-issued photo ID; an original copy of
the birth certificate plus school identification, or school yearbook).
- In the event the parent has a different last name and/or address from the child, documentation is needed
to prove the relationship (divorce or marriage certificate; custodial or adoption papers; an original
copy of birth certificate).
- A unique single exception would be an emancipated and/or married minor who presents positive
proof of their legal emancipation and/or marriage. The consenting parent must simply fill out the form
below, and attach a photo of your State or Federal identification. All items must be submitted,
and approved before any body art procedure is scheduled! You are required to bring these items into the studio
for verification. With a signed release from a parent or verified legal guardian, minors may be eligible for
• Earlobe or Ear Cartilage
Other piercings are potentially dangerous, unethical to perform, or problematic to heal on minors whose bodies are still growing and changing. Under no circumstances is it acceptable for any of our piercers to perform a piercing
on the nipples or genitals of an individual under 18 years of age!
